GoVets Credit Reference Form
GoVets has received a request for business credit terms and all designated credit references (specified by the Applicant) must fill out the following form.
NOTE: Here are some requirements for the following form:
- Individual submitting a Credit Reference on behalf of the Application Company must be a Sr. Level Manager that is able to submit the credit reference and company information on behalf of the company and credit reference organization.
- Credit Reference organization must have a viable Corporate Website.
- Credit Reference applications cannot include E-Mail addresses from Gmail, Yahoo, Outlook or other free E-Mail services. E-Mail address used for the Credit Reference must include the Company name within the Domain. Example: Application from UTools, Inc. must have an E-Mail that is similar to JohnSmith@UTools.com. E-Mail from JohnSmith_UTools@Gmail.com (or similar) will be rejected.
- Credit Reference must be located in the United States.
- Credit Reference must be in business for at least 2 years.
